Direct Support Professional/Caregiver/DSP
Assists people supported with activities of daily living, including bathing, dressing, grooming, toileting and transferring while encouraging people supported to perform as much self-care as possible. • Fostering positive relationships between individuals served and their housemates. • Drives people supported to daily appointments, work, day programs, recreation events and stores as needed. • Maintains records in accordance with company policy and procedures. This includes documentation related to service delivery, progress notes, change in health and emotional condition or other significant events. • Completes all documentation accurately and timely. • Ensuring client safety and maintaining a safe environment. • May assist people supported with medication or assists or supervises people supported who self-administer medication. Completes medication records and counts. • Maintains a clean, safe, and orderly environment for the people supported. Performs general housekeeping and teaches/supports client to perform housekeeping. • Understands and follows proper physical safety and emergency protocols. • Encouraging self-help activities. • Reporting changes in client's condition or family situation to the office. • Performing home management functions such as light housekeeping, laundry, bed making, and cleaning. • Planning meals, shopping for groceries, preparing and serving food/meals, feeding and clean-up. • Accompanying client to scheduled appointments. • Understands and utilizes adaptive equipment including wheelchairs and belts/locks, Hoyer lifts, etc. and be able to securely load and unload clients. • Completes all trainings including new hire, on the job and annual trainings within the designated timeframe. • Other duties as assigned.
